Register with our practice
Print, complete and sign the registration form then bring into the surgery between 8.30am – 6.30pm Monday – Friday . You will also need to provide 2 forms of identification, one photographic and one to confirm your address.
*All applications need to be considered, so it is important to please contact the surgery 2 days after reception receive the form to check on your application. Please feel free to either call in or telephone0151 652 4281 and select option 2 for patient enquiries.*

Named GP
From 1st April 2015, GP Practices are required to allocate every patient with a named GP. This does not affect your treatment or care at the Practice, and you can continue to see any GP of your choice as normal.
