NHS Health Checks (Healthcare Assistant) these are a FREE healthy heart check for anyone aged 40-74 who hasn’t had one on the last 5 years and who does not have an existing long term condition – please book via Reception (you will require blood tests – please book via Reception)
The NHS Digital Weight Management Programme supports adults living with obesity who also have a diagnosis of diabetes, hypertension or both, to manage their weight and improve their health.
It is a 12-week online behavioural and lifestyle programme that people can access via a smartphone or computer with internet access
About the NHS Digital Weight Management Programme
The NHS Digital Weight Management Programme is a free, 12 week online programme that provides access to weight management plans.
Upon referral, you will receive a text message text message within 2 working days from NHS WMP inviting you to the programme. Then you can choose a weight management plan that will help you develop healthier eating habits, achieve activity goals and lose weight. Each plan provides recipes and nutrition advice, wellbeing support and tips to boost your activity levels. The Programme is accessed online, so it can be completed anywhere, in your own time.
It can be tough to make changes but with our support you can take control of your weight and improve your overall health.
Who can be referred?
You are eligible for the programme if:
you are age 18 or over
you have a BMI of 30 or more. The BMI threshold is 27.5 for people from black, Asian, and ethnic minority backgrounds, who are at an increased risk of some conditions at a lower BMI, such as type 2 diabetes
you have a diagnosis of diabetes (type 1 or 2), high blood pressure, or both
You will need regular access to the internet, using a smartphone or computer, to complete the NHS Digital Weight Management Programme.
If you do not meet the eligibility criteria, you can access online support from NHS Better Health, or speak with your GP Surgery who may be able to offer an alternative service.
